Aptiva Health Physical therapy - Middletown

Louisville's only Physical Therapy and Athletic Training clinic embedded inside a performance gym. Co-located with MPower6 at 401 N English Station Road.

Middletown is Aptiva's dedicated Physical Therapy and Athletic Training facility — purpose-built for sports rehabilitation, post-surgical recovery, and return-to-play care. We share a building with MPower6 Performance Gym, which means our patients have access to a real continuum of care: clinical rehabilitation in our PT clinic, transitioning into strength and conditioning, sport-specific performance work, and recovery — all under one roof. Whether you're a high school athlete coming back from an ACL reconstruction, an active adult recovering from rotator cuff surgery, or a competitive athlete working on sport-specific performance, this location is built for what comes after injury — not just the rehab phase, but the return.

Rehab under one roof in Middletown

Rehab to Return-to-Play — Under One Roof

Most physical therapy clinics operate in a clinical bubble. A patient finishes their course of PT, gets cleared, and is told to "go back to working out" — but the gap between rehab exercises and return to actual sport or strenuous activity is exactly where re-injury happens. Patients who don't have access to a bridge from clinical rehab to real-world strength and performance work often plateau, get re-injured, or never quite return to their pre-injury level.

Our Middletown location is built specifically to close that gap. Aptiva Physical Therapy in Suite 1A is co-located with MPower6 Performance Gym — Louisville's premier performance facility — in the same building. That means a patient's care path can look like this:

1. Clinical Rehabilitation (Aptiva PT) Initial evaluation, hands-on physical therapy, manual therapy, exercise progression, post-surgical recovery. Delivered by Simon Hoehn, PT, DPT and Zach Clark, PT, DPT, LMT.

2. Sports Science & Athletic Training (Aptiva AT) As the patient progresses, our Head of Athletic Training and Sports Scientist, Aurelio Puga, LAT, ATC, supports return-to-play planning — sport-specific testing, performance analytics, return-to-sport progressions, and evidence-based return-to-play decision making.

3. Performance Training (MPower6, optional) For patients moving from rehab to performance, MPower6's facility provides the equipment, space, and trainers needed for sport-specific strength and conditioning. State-of-the-art weight floor, functional performance area, Hyperice Recovery Lounge, sauna, smoothie bar. Available with MPower6 membership.

4. Return-to-Play Final clearance, return-to-sport testing, and ongoing maintenance — coordinated across the PT clinic, athletic training, and MPower6 as appropriate.

The result: no gap between clinical recovery and real-world readiness. Patients don't have to drive to multiple facilities, repeat their history to teams that don't talk to each other, or guess at the transition point between "rehab complete" and "ready for sport."

Important: MPower6 membership is not required to receive PT or athletic training services from Aptiva. The two businesses operate independently. The co-location is a convenience and a clinical advantage for patients who want it — but PT patients who prefer to do their post-rehab work elsewhere are welcome to do so. The continuum is an option, not a requirement.

FAQ #3

Question: What services are offered at Aptiva Middletown?

Answer: Aptiva Health Middletown is a dedicated Physical Therapy and Athletic Training facility. Services include sports injury rehabilitation, orthopedic physical therapy, post-surgical recovery, return-to-play evaluation, athletic training, sports science consultation, performance optimization, injury prevention programs, and manual therapy. The location does not offer orthopedic surgery, spine care, interventional pain, or imaging - those services are available at our other Aptiva locations. Most Middletown patients are referred for PT after evaluation at another Aptiva location, or come directly for sports performance and return-to-play work.

FAQ #4

Question: Do I need a referral for physical therapy?

Answer: Kentucky allows direct access to physical therapy - most patients can self-refer without a physician referral. Some insurance plans (particularly HMOs and certain Medicare Advantage plans) may require a referral for coverage. Call 502-323-3639 to confirm your plan's specific requirements before scheduling.

FAQ #5

Question: Do I need a gym membership at MPower6 to be treated at Aptiva Middletown?

Answer: No. Aptiva Health and MPower6 Gym are separate businesses sharing the same building. You can receive physical therapy and athletic training services at Aptiva without any gym membership. The co-location is a convenience and a clinical advantage when patients progress to performance training - but it is not a requirement. Patients who would benefit from gym-based performance work after rehab can choose to join MPower6 or use any other facility.

FAQ #6

Question: What kinds of injuries and conditions do you treat?

Answer: Aptiva Middletown treats sports injuries (ACL, meniscus, rotator cuff, sprains, strains, tendon injuries, overuse injuries), orthopedic conditions (post-operative rehabilitation following knee, shoulder, hip, and spine surgery), athletic performance concerns (return-to-play, return-to-sport, sport-specific deficits), and general physical therapy needs for active adults. Most patients fall into one of three groups: post-surgical patients returning to function, athletes recovering from injury and preparing to return to play, and active adults working on performance and injury prevention.

Question: Who is Aurelio Puga and what does a Sports Scientist do?

Answer: Aurelio Puga, LAT, ATC is Aptiva's Head of Athletic Training and Sports Scientist. As a Licensed Athletic Trainer and Certified Athletic Trainer, he provides athletic training services, injury prevention programs, return-to-play protocols, and performance optimization for athletes. The Sports Scientist designation reflects work that goes beyond standard athletic training - including performance analytics, sport-specific testing, evidence-based return-to-play decision making, and coordination with athletic departments and coaches. He works closely with our physical therapists to provide a coordinated continuum from rehab through return to competition.

FAQ #8

Answer: No. Aptiva Health offers physical therapy at most of our locations - including our Louisville Central, East, Downtown, Concussion & Sports Medicine, Mt. Washington, and Elizabethtown clinics, our Lexington Physical Therapy location, and our other locations across Kentucky and Indiana. The Middletown facility is distinctive because it is our only dedicated Physical Therapy and Athletic Training location (no surgical or medical specialty services offered on site), and the only one co-located inside a performance gym.

FAQ #9

Question: What insurance plans does Aptiva accept?

Answer: Aptiva Health accepts most major commercial insurance plans, Medicare, Medicare Advantage plans, auto insurance carriers, and workers' compensation. Specific plan participation may vary - call 502-323-3639 to confirm coverage for your plan before your visit. Cash-pay options are available for patients without insurance.
