Rehab to Return-to-Play — Under One Roof
Most physical therapy clinics operate in a clinical bubble. A patient finishes their course of PT, gets cleared, and is told to "go back to working out" — but the gap between rehab exercises and return to actual sport or strenuous activity is exactly where re-injury happens. Patients who don't have access to a bridge from clinical rehab to real-world strength and performance work often plateau, get re-injured, or never quite return to their pre-injury level.
Our Middletown location is built specifically to close that gap. Aptiva Physical Therapy in Suite 1A is co-located with MPower6 Performance Gym — Louisville's premier performance facility — in the same building. That means a patient's care path can look like this:
1. Clinical Rehabilitation (Aptiva PT) Initial evaluation, hands-on physical therapy, manual therapy, exercise progression, post-surgical recovery. Delivered by Simon Hoehn, PT, DPT and Zach Clark, PT, DPT, LMT.
2. Sports Science & Athletic Training (Aptiva AT) As the patient progresses, our Head of Athletic Training and Sports Scientist, Aurelio Puga, LAT, ATC, supports return-to-play planning — sport-specific testing, performance analytics, return-to-sport progressions, and evidence-based return-to-play decision making.
3. Performance Training (MPower6, optional) For patients moving from rehab to performance, MPower6's facility provides the equipment, space, and trainers needed for sport-specific strength and conditioning. State-of-the-art weight floor, functional performance area, Hyperice Recovery Lounge, sauna, smoothie bar. Available with MPower6 membership.
4. Return-to-Play Final clearance, return-to-sport testing, and ongoing maintenance — coordinated across the PT clinic, athletic training, and MPower6 as appropriate.
The result: no gap between clinical recovery and real-world readiness. Patients don't have to drive to multiple facilities, repeat their history to teams that don't talk to each other, or guess at the transition point between "rehab complete" and "ready for sport."
Important: MPower6 membership is not required to receive PT or athletic training services from Aptiva. The two businesses operate independently. The co-location is a convenience and a clinical advantage for patients who want it — but PT patients who prefer to do their post-rehab work elsewhere are welcome to do so. The continuum is an option, not a requirement.
