Where does Aptiva Health offer physical therapy?

Aptiva Health offers physical therapy across Kentucky and Indiana, including Louisville, Lexington, Elizabethtown, Indianapolis, and Mt. Washington. Call to confirm the location most convenient for you.

Do I need a referral to start physical therapy?

In many cases, no. Kentucky and Indiana both allow direct access to physical therapy, meaning you can begin treatment without a physician referral, subject to some limits. Call us and we will confirm what applies to your situation and your insurance.

What conditions does physical therapy treat?

Our physical therapists treat orthopedic and sports injuries, neck and back pain, spine conditions such as herniated discs and sciatica, post-surgical recovery, sprains and strains, arthritis and tendonitis, pelvic floor and women's health concerns, and balance, dizziness, and concussion-related issues.

Does Aptiva Health offer pelvic floor and women's health physical therapy?

Yes. Our team includes therapists with advanced training in pelvic floor and women's health, treating concerns such as pelvic pain, incontinence, diastasis recti, and pregnancy- and postpartum-related conditions.

Do you have Spanish-speaking physical therapists?

Yes. We have bilingual physical therapists who treat patients in both English and Spanish so Spanish-speaking patients can receive care in their preferred language.

What is dry needling?

Dry needling is a technique in which a thin, sterile needle is inserted into a tight muscle or trigger point to relieve pain, reduce muscle tension, and improve movement. It is often combined with manual therapy and exercise as part of a physical therapy plan.

Meet our Physical Therapy Director: Simon Hoehn, DPT

Our Physical Therapy Team

Simon is the Director of Physical Therapy at Aptiva Health. He specializes in injury prevention, neuro-restorative rehabilitation, and post-operative physical therapy. Simon is also a professionally trained business coach and former college football player. He brings a wealth of knowledge to the physical therapy team and ensures the highest level of patient care and customer service at Aptiva Health.

Adam is a former collegiate wrestler and is an avid climber and recreational athlete. Adam focuses on sports-related injuries and brings his own athletic background to help efficiently diagnose a variety of musculoskeletal conditions. Adam is certified in dry needling and specializes in manual therapy and hands-on techniques.

Tori is passionate about treating barbell athletes, weight lifters, and cross-fit athletes in addition to weekend warriors. Tori has also received advanced training and certifications in the Graston Technique to treat musculoskeletal injuries and conditions.


Jessica has been a physical therapist for over 12 years treating orthopedic conditions with specialized interest in women's pelvic health and wellness. She also has advanced training in the roles of female hormones through menstruation, maternity, and menopause.

Monica is a physical therapist who specializes in soft-tissue restriction, myofascial release and movement related dysfunction. She is a bi-lingual, speaking fluent Spanish and English. Her experience in treating patients with a variety of acute and chronic injuries has made her a dynamite fit for our team and our patients.

Jared is a graduate of Bellarmine University and he comes to Aptiva with a strong background in vestibular therapy. He will be leading our Sports & Concussion Rehab department and is working closely with our Neuropsychologist and Athletic Trainer to assist with ‘Return to Play’ for local athletes, especially those who suffer from Concussion and Sports Related Injuries.


Helen takes a holistic approach to physical therapy. She is fluent in multiple languages and takes a hands-on approach to patient care. Whether its manual therapy or other hands-on techniques, Helen makes sure each patient receives the most out of their physical therapy sessions.

Rachel forrester

Rachel is a dedicated physical therapist with over 9 years of clinical experience treating orthopedic conditions and sport related injuries, with advanced certifications in manual therapy and trigger point dry needling.

Wendy, a Louisville native and University of Louisville Physical Therapy graduate, specializes in treating musculoskeletal conditions like fibromyalgia and osteoarthritis. She’s known for her patient-centered approach and commitment to helping patients achieve their goals. Outside work, Wendy enjoys family, sports, classic cars, and caring for her seven dogs.


Emanuel Mora, PT, DPT, is a bilingual physical therapist in Lexington, KY, who specializes in outpatient care. With a Doctor of Physical Therapy degree from the University of Kentucky, Emanuel is passionate about supporting Spanish-speaking patients and addressing language barriers in healthcare. He combines technical skills with a patient-centered approach to help his patients regain mobility and enhance their quality of life.

Zachary Clark, a DPT graduate from the University of St. Augustine in 2022, has nearly two years of experience. He enjoys golf, rec sports, and working with overhead throwing athletes due to his baseball background. He is eager to learn from colleagues, particularly in concussion management and injury prevention, to provide individualized patient care.

Aurelio is originally from San Diego, CA and earned his BA Degree in Exercise Science from the University of Louisville, and a Master’s degree in Athletic Training form Spalding University.  His most recent experience was with Lou City FC as the assistant to the head athletic trainer.


Evan Hall, PT, MPT

Evan Hall, PT, is Lexington Physical Therapy Manager, where he is passionate about supporting the organization’s growth while contributing to mentorship and educational development for clinicians.

Clinically, Evan has a strong interest in treating shoulder injuries,  lumbar spine pathologies and working with complex cases.

Jessie Edward, PTA

Jessie is our Physical Therapist Assistant in our Lexington, Kentucky office. Jessie focuses on treating musculoskeletal and orthopedic injuries and conditions. She works directly with our physical therapists to develop individualized treatment plans for each patient.


Katie Germak, PTA

Katie is a Physical Therapist Assistant at our Newburg location in Louisville. She uses a hands-on approach with patient care and is well versed on many treatment techniques. Katie is Herman and Wallace Pelvic Floor level 1 certified allowing her to work with patients who are experiencing pelvic pain, incontinence, diastasis recti, and more. She is passionate about empowering clients to achieve their goals and to regain their confidence in daily and functional tasks.

Ann Mills, PTA

Ann is a Physical Therapist Assistant in our Physical Therapy Department. Ann has a passion for treating musculoskeletal injuries and conditions and working with athletes. She takes a very hands on approach to patient care and believes strongly in providing a friendly and open atmosphere for patients.

Marcus is a Physical Therapist Assistant in the Physical Therapy Department and a Certified Strength and Conditioning Specialist through the NSCA. He is a former college football player who has also competed and coached in powerlifting. Marcus is experienced in treating musculoskeletal conditions and post-operative rehab with a variety of interventions.

Merchelle Stokley is a licensed massage therapist with over 13 years of experience, specializing in Swedish, deep tissue, trigger point, and relaxation techniques. She tailors each session to individual needs, helping clients manage pain, recover from injuries, and reduce stress. Guided by her philosophy that healing begins when the body feels safe and cared for, Merchelle is dedicated to restoring balance and overall well-being.

Dovie is a dedicated Physical Therapist Assistant with 13 years of experience in Hardin and Nelson County, specializing in outpatient, home health, and rehab hospital settings. She is committed to personalized care, using manual therapy, therapeutic exercises, and patient education to help individuals recover from a wide range of injuries, including sports, orthopedic, neurological, and post-operative conditions.


Kim is a licensed Physical Therapist Assistant with 25 years of experience, including nearly two decades in outpatient orthopedics. She has specialized in post-surgical rehabilitation, sports injury recovery, aquatic therapy, and orthopedic bracing, while also treating patients in hospital and rehab settings. Known for her hands-on approach and patient education, Kim combines clinical expertise with personal experience managing chronic pain to deliver compassionate, individualized care.

Jessa Berry, a 2014 Hanover graduate with a BA in Exercise Science, later earned an Associate Degree as a Physical Therapy Assistant from Ivy Tech. With seven years of experience, she specializes in joint post-operative care, particularly shoulder rehabilitation, and enjoys helping athletes return to sports while balancing her love for the outdoors and family time.

I was on vacation in Florida when a balcony collapsed on me. Luckily it didn’t fall all the way through but it fell at least 2 feet, causing an injury to my left shoulder, lower back and neck which created so much pain for me.
